Role Overview

The Registered Nurse, Virtual Care provides high-quality clinical support to patients and caregivers through virtual care platforms. Reporting to the Nurse Cohort Manager, this role is responsible for delivering consistent, patient-centered care, maintaining excellent clinical documentation, and collaborating across care teams to ensure continuity and quality outcomes.

This is a fully remote role designed for nurses who thrive in a fast-paced, mission-driven virtual care environment.


Schedule & Employment Type

  • Full Time, Monday through Friday, Exempt

  • Shifts Available 8:00 am to 4:30 pm PST

  • Some Holiday’s and weekends may be required

  • Consistent schedule availability and adherence are required


Roles and Responsibilities

The virtual nurse will work closely with patients, caregivers, care coordinators, the PocketRN team, and our partners to ensure continuity of care and facilitate cross-functional communication. The virtual nurse will play a vital role in providing high-quality healthcare services to patients virtually. Their roles and responsibilities will include, but are not limited to:


Clinical Care & Patient Support

  • Deliver virtual nursing care using evidence-based clinical judgment and the nursing process

  • Conduct comprehensive health assessments, triage patient concerns, and provide clinical guidance across multiple care settings

  • Promote patient health, safety, and wellness through proactive outreach and education

  • Provide education and clinical support to family and professional caregivers


Communication & Care Coordination

  • Build strong rapport with patients, caregivers, care coordinators, and partner organizations in a virtual environment

  • Communicate clearly, compassionately, and professionally with all stakeholders

  • Identify and escalate urgent or concerning clinical findings according to established protocols

  • Collaborate with internal care teams to ensure timely follow-up and continuity of care


Documentation & Quality

  • Maintain accurate, timely, and thorough documentation in the electronic health record (EPIC)

  • Ensure documentation meets PocketRN clinical standards, quality expectations, and regulatory requirements

  • Adhere strictly to HIPAA and patient privacy standards in all virtual interactions


Professional Practice & Team Engagement

  • Follow PocketRN workflows, clinical guidelines, and escalation pathways

  • Actively participate in coaching, performance feedback, and ongoing education

  • Contribute to continuous improvement initiatives and support a collaborative team culture

  • Demonstrate accountability for schedule adherence, attendance, and professional conduct


Requirements:


Education

  • Graduate of an accredited school of nursing

Experience

  • Minimum of 3 years of direct patient care experience

  • Experience in acute and primary care.

Licensure

  • Licensure: Must have a current valid California license. Does not need to reside in the state of California

  • Strong clinical judgment, critical thinking, and assessment skills

  • Excellent verbal, written, and asynchronous communication skills

  • Ability to deliver empathetic, patient-centered care in a virtual setting

  • Comfortable working independently while remaining engaged with a remote team

  • Adaptable and eager to learn in a fast-paced health-tech startup environment


Technical & Work Environment Requirements

  • Proficiency with EHR systems (EPIC preferred), Google Workspace tools

  • Reliable high-speed internet connection (minimum 50 Mbps)

  • Quiet, private home workspace suitable for HIPAA-sensitive video calls


Professionalism & Communication

  • Able to demonstrate effective interpersonal, oral, written, and listening communication skills.

  • Able to communicate effectively, in English, over the phone, video conferencing, and in writing. To be successful in a virtual environment, must be proficient in asynchronous communication

  • Provides courteous, empathetic, and clear communication to patients even in stressful scenarios. A desire to go above and beyond for patients. Patient-Centered: Demonstrates patient-centered/patient-valued behaviors. Focuses on patient needs, patient rights, confidentiality, and religious and cultural preferences.

  • Must be able to work effectively in a remote environment by taking initiative, go above and beyond to delight patients, be actively involved in process improvement, and be reliably available during determined shifts.

  • Able to learn new systems and workflows quickly and speedily adapt nursing in the virtual care start-up environment.
