Position Summary & Responsibilities:
Position Summary:
The Kansas Soldiers’ Home at Fort Dodge is looking for a dependable individual to work under the Director of Nursing as an Informatic Nurse, Monday thru Friday in our long-term care unit. We are looking for someone who helps grow our nursing team and provide the best care to our nations heroes.
Job Responsibilities may include but are not limited to the following:
Nursing Care. Responsible for the provision of indirect care to groups of residents. Monitors physical, mental, and environmental conditions to ensure that resident’s needs are met. Makes rounds with physicians, communicating. Delegates appropriate tasks to licensed staff including LPN’s. Will be on-call as required by management.
Coordination of Care, Education and Instructing. Coordinates with other disciplines to provide needed services as identified in the Individual Plan of Care. Provides staff training through teaching and instructing classes in long term care as part of the training program for nurses, nurse aides, allied health practitioners and other disciplines as appropriate. Instrumental in developing appropriate education to address discrepancies between nurse aide ADL charting and current resident condition.
Nursing Assessment. Is the Minimum Data Set (MDS) coordinator for the assigned neighborhoods / units. Performs mandatory nursing assessment, as required by State Regulation and implements that assessment into an individual plan of care, by observation, medical/social history and analyzing appropriate records. Incorporates the nursing approach to problem solving for the resident, reviews and revises the nursing care plan in a timely and appropriate manner. Will acknowledge Care Area Triggers and perform Care Area Assessment to develop a resident-specific care plan based on identifiedproblems, needs, and strengths with a goal to promote the highest practicable level of functioning for the residents.
Advocate / Resource. Advocate for the residents by serving as a liaison between resident and care or service providers. Interacts with residents and families in a therapeutic manner, providing guidance, direction, and support to encourage residents and promote healthy behavior and have a positive experience.
Coordination of RAI Process and EHR/Information Technology Development. Provides training when appropriate. May need to adjust hours of work in order to meet with the staff of all three shifts. Involves both functional and administrative responsibilities. Primary responsibility is to ensure MDS and ADL coding accurately reflect the current condition of residents. If there appears to be conflicting information, the Informatic Nurse works with the MDS Coordinator to negotiate with the IDT and ensure accuracy of information. Monitor completion of the CAA process and will sign at V0200B, C to indicate the CAAs triggered by the assessment have been reviewed and a decision recorded whether or not to proceed with Care Planning. Develop necessary PN templates and issue tracking processes through IT infrastructure. Develop reports, spreadsheets and graphics to reflect current state of quality measures/historical data and present information to IDT for review and planning. Participate in planning processes towards higher quality ratings.

At time of appointment the employee must possess a Kansas license/temporary permit, or be given authority to practice as a Registered Nurse (RN) by the Kansas State Board of Nursing.
Minimum Qualifications

Knowledge of: 1) professional nursing theory and practice, and 2) internal working relationships of the facility staff

Ability to: 1) apply nursing techniques as directed to patient care situations, 2) coordinate the services of a staff of professionals to provide appropriate nursing care to a group of patients, 3) follow specific or general procedures and directions, 4) prepare and maintain records, charts and reports, 5) maintain a helpful and sympathetic attitude towards patients under trying conditions and to maintain harmonious working relationships with subordinates and supervisors, and 6) instruct staff of professional, paraprofessional and non-professional personnel responsible for the care and treatment of the physically ill and aging population.

As authorized by the 2018 Kansas Legislature, effective July 1, 2018, K.S.A. 75-4362 is amended to designate all positions with the Kansas Commission on Veterans’ Affairs office, to include both the Kansas Soldiers’ Home and Kansas Veterans’ Home as Safety Sensitive positions and subject to the drug testing program. All applicants with a conditional offer of employment on or after July 1, 2018, shall be required to submit to a drug screening test.
