Job Description

I. JOB SUMMARY

The Psychiatric Mental Health Nurse Practitioner (PMHNP) provides direct care in accordance with established policies, procedures, and protocols of the healthcare organization, while complying with laws and regulations in maintaining patient information. This position leads the coordination of care for primary care patients by applying effective decision making skills based on patient/family healthcare needs and priorities.

I. ESSENTIAL JOB RESPONSIBILITIES
• Responsible for providing psychotherapy and medication evaluation and management to patients in primary care.
• Offer alternative methods for meeting such as telehealth and group therapy to reduce barriers for patients.
• Create, implement, and manage screenings and work with community providers to ensure screening consistency and referral patterns.
• Develop and coordinate education for providers
• Serve as a resource and guidance for community education for group support programs.
• Develop therapeutic relationships with patients and families promoting decision making based on the assessment of patient medical histories, symptoms, examinations, diagnosis, conditions, and treatment.
• Communicate and collaborate concisely and continuously with clinical team, including physicians, both verbally and in documentation focusing on patients identified problems and needs by implementing and monitoring patient care plans, as well as recording and communicating patient conditions as appropriate, to optimize their health outcomes.
• Assesses and coordinates patient's discharge planning needs with members of the healthcare team.
• Competently applies collaborative teamwork and leadership as a member of an inter-professional team to coordinate patient centered care for quality and outcomes of care processes by:
o Utilizing information, resources, and technology
o Provide patient centered care nursing processes including patient/family, evidence based practices, safety, and process improvement
• Exemplified professionalism; applies lifelong learning; contributes to the science of nursing; participates in governance; shares knowledge to expand the scale and scope of best practices with others; builds the ethical, moral, and regulatory base for best practices and processes of patient and unit care.
• Performs other duties in line with current PMHNP scope of practice.

I. JOB QUALIFICATIONS

Required:
  • Master's degree in Nursing
  • Current Advanced Practice Registered Nurse license to practice in Kansas
  • Current Basic Life Support for Healthcare Providers
  • 3-5 years working as a psych Nurse Practitioner in an outpatient or inpatient setting
  • PMHNP Certification

Preferred:

  • Experience with mental health crisis management
